I was wondering how high people have gone with their meet-me conferences without the addition of an HW bridge. I have a customer that wants to hit 46. Customers current setup is three CM's all 3845's.

3845 wont help if they use pvdm2, max participants per conference is 8.
You can use IPVMS to get more users, max 128, check srnd media resources section for more information.
Is there any reason he wants 46?
I have been in large conferences and even with 8 people is hard to listen or speak i cannot imagine 46.

Just a note on the maximum number of participants in a PVDM2 sponsored conference: The upper limit has been increased to 32.
The SRND has been updated to now state:
"A conference based on these DSPs allows a maximum of 8 participants. When a conference begins, all 8 positions are reserved at that time. Starting with Cisco IOS Release 12.4(15)T, this limit on the maximum number of participants has been increased to 32."
