International digits

I am researching CUCM7, want to get some sense how telco(s) to deal with the international calls.

my question is, with a voice gateway, when international call (caller) hit the gateway (assuming the gateway is in USA or any place in the world), how many digits we can see for the calling number (the international number)? country code? 011+country code for any caller? 00+country code for Europe caller? 0011+country code for Australia caller?

With ISDN, there is an indication that it is an international call, then the calling number starting with country code.

You never see the international call prefix or the + symbol.
