unity version 4.1(1) went to failover mode, but did not find any log mentioned ports were locked or something failed, we have 32 ports and will be enough for users, any idea will be appreciated.

Failovers are usually not straight forward when trying to track down. The following doc explains all of the tracing required prior to a failover event happening in order to even begin looking into root cause (for the situations that aren't obvious). It requires cisco.com login.
