Do you have to use an access code to get an outside line? Can you configure the system to not use it & where would I find documentation on that?

Yes, the steering digit is required because the SPA9000 is a PBX and can have up to four different potential outbound routes using SIP ITSPs or PSTN lines. The steering digit is used to define the trunk to use.
If your user does not want to dial a steering digit, you can configure the SPA9000 to automatically add an appropriate steering digit based on the number dialed. This is described in the SPA9000 Admin Guide. Search on substitute in order to find number substitution in the About Dial Plans section.
Basically what you'll do is add a 9, for example, when 10-digits are dialed with: <:9>xxxxxxxxxx

No, you don't need an access code, but you do need a steering digit to seize the appropriate trunk to make the outbound call.
Consider using the SPA9000 Setup Wizard for your configuration, makes the configuration a lot simpler.
The steering digit is defined in the SPA9000 Admin Guide:
