CME - conference call with SIP trunks

I have been having difficulty creating a conference call when using SIP trunking. Concalls to internal phones - no problem. Making phone calls to the PSTN via SIP trunk - no problem. But make a call to the PSTN via SIP, hit concall buttong, call an other PSTN call via SIP and hit concall button and the operation can not complete. Anyone have this actually working?

Make sure all dial-peers has dtmf-relay set to rtp-nte

Add g711 codec to profiles for outbound dial-peers

Select the "MTP Required" button on the SIP trunk configuration
