2nd username and 2nd password on login screen

OK, another dumb question from a newbie,,,,, I was working on setting up smart tunnels for some of my users and I was editing the web page under "clientless ssl vpn access" - "portal" - "customization" and somehow I added the "2nd username" and "2nd password" fields to the logon form, how do I turn those off? any one know? I can't figure it out. Thanks in advance for your help.

Well, I fixed my own problem. Somehow I put this command in the ASA.

no secondary-authentication-server-group LOCAL use-primary-username

Not sure how I did it but when I deleted it the problem went away.
