Erractic Call Behavior

We have been experiencing erratic call behavior on our ip phones since sometime now. For every 1 out of 3 incoming calls, the call from the outside goes directly to the user's voicemail or gets d/c even though the phone was not busy. We get an email notification from our provider (speakeasy) for an incoming call and at times we see the incoming notification but the phone does not ring!

Sometimes just taking the receive off hook and putting it back gets the call to ring in.

The same is experienced while dialing internal extensions too.

Another instance is when a previous call has just ended on phone A and if you happen to dial the same phone within the next 5-6 seconds, it goes to the user's voicemail

Can someone please explain why this is happening and a solution for the same?

I assume this is a SIP call, if so, we will need a debug ccsip messages first.

Can you also provide a show run?

Does your provider send from multiple IP addresses?

Honestly, we could try to do this here, but this sounds like a serious issue that would be better handled with TAC.  If you open a case, would you mind sending me the case number?
