Diagnosing SPA400 connectivity issue

I am adding an SPA400 to an existing SPA9000/SPA922 configuration to add connectivity to POTS.

However, with no matter how much investigation, I cannot figure out why Line 1 is showing as 0V.

Cisco documentation only talks about checking the wiring, but I am using a known good RJ11 -> BT Phone cable.

I have tried default line impedance (600) and also the BT defined (http://www.sinet.bt.com/ Doc 351 / Figure 1) of 370/620/310nF but either way the line is still showing as 0V.

Any tips/suggestion on how to proceed would be welcomed.
