NAM Upgrade Problem

While I was upgrading NAM 2 from ver 3.6 to 4.1 I found that the connection is (Flapping) after the upgrade, then I found the following error on the core. (The NAM module is restarting)

000947: 12w1d: %SNMP-5-MODULETRAP: Module 2 [Down] Trap

000948: 12w1d: %OIR-SP-3-PWRCYCLE: Card in module 2, is being power-cycled 'off (Module not responding to Keep Alive polling)'

000949: 12w1d: %C6KPWR-SP-4-DISABLED: power to module in slot 2 set off (Module not responding to Keep Alive polling)

After checking the error meaning, I have found that the workaround is to restart the module many times!!

But the problem never solved, the problem only is resolved if I restarted the NAM in the (cf:1) mode, otherwise the module keeps restarting.

The following are the output of the show ver:

Could anyone help to resolve this problem?

Can you please post a show module from the switch and a show version from the switch?

Can you also run the following memory test from the switch:

hw-module module 2 reset mem-test-full

The output of the show module on the switch:

Hi there,

Were you ever able to find a resolution for this issue?  I'm having the exact problem with my NAM-2 module and I can't figure out any steps.  Is this a bug in 4.1?

i have paused the upgrade till i find an answer, but i will try again in the coming day. i still didn't find an answer for the situation.

i'll inform u if i succeeded in my upgrade...
