Call Forwarding Broke with Call Manager Express

We are running CME. Our SIP trunking provider recently changed our SIP PBX and broke a bunch of stuff (number of digits sent, DTMF). The most recent thing I found that's broken is ability to forward all calls to say a cell phone. When I dial the Cisco phone - the cell phone rings but no voice can be heard in either direction. Anyone have a thought of what I might have happened and how to resolve? My voice services section looks as follows:
