Inconsistent Dial by Extension behaviour

Deployed a CM BE 6.x. Having an inconsistent problem when users access the AA. The problem is when they attempt to dial by extension from the main greeting, ocassionally an error is given indicating that the input was not recognized. However from the main greeting a user generally can get to an extension by just dialing after the AA starts. Any ideas as to what can contribute to this kind of behaviour?

If this is for external callers check your DTMF configuration is correct on your gateway

Thanks for the response. Due to some other problems I had in the translation profile, I simply created a voip DP for each incoming number then send that call to the CM. The CM would then do a translation pattern to the extension for the AA.

For each of the voip DPs created the dtmf setting is set to dtmf-relay h245-alphanumeric.

The thing that makes it tricky is that other options (from caller input) are accessed fine. Just that dial by extension provides inconsistent results.
