Does anyone know if it's possible to get new ESW+UCSS for CM when upgrading from 4.x to 7.x, if the customer didn't have ESW+UCSS before.

I think I figured it out. There's no upgrade SKU for 4.x->7.x, you simply get new 7.x (same price) and get ESW+UCSS with it.
