problem with QOS Report

Dear All,

Madhab Here...I am facing a problem with My IP Telphony Network.

I am trying to get the QOS Report of Call in CUCM 6.1.3

But I am getting only NA in QOS columns though I defined all the Values of QOS  through Report Config -> Define QOS.

I am attachinf my current configuration of QOS.

Please help me out ASAP.
