524SG is getting an SCCP registration failure - not visible in cca or cme

I have a 524SG phone that was working, in default, waiting for me to customize. We plugged it in and it got an extension. Now there is no ephone or ephone-dn with the phones information or any evidence that it ever registered at all.

How can I get this phone back?

What troubleshooting steps can I try?

What cli debugs will tell me issues with phone registration?

I tried the phone debug in CCA but it only shows debug info for registered phones.

If you add the phone in under Configure -> Telephony -> Users Extensions, you should see it under Phone Debug Log.  Do that, and post the logs here and I will help you with why it isn't registering.
