FXO issue on outgoing call

Wondering if anyone could provide some thoughts on this issue.  I have 4 FXO lines connected to Verizon FIOS service.  This installation has been working fine for over 2 months and all of sudden we are experiencing outbound calling issues.   Every some number of calls, when they dial the number the UC520 picks a line and then the user is presented with a low tone and they hear dial-tone.  I don't know if I missing something on my voice-port configuration or is this something that is a Verizon issue.

All outbound POTS ports are configured like this:

voice-port 0/1/0

trunk-group ALL_FXO 64

supervisory disconnect dualtone mid-call

timeouts interdigit 3

timeouts call-disconnect 3

timeouts wait-release 5

connection plar 501

description xxx.xxx.xxxx

caller-id enable

And you are talking about outbound calls right? It could be that you are matching an outbound dial-peer but not sending additional digits, therefore you get dialtone from the Telco, since you are basically just seizing the line. I would check the dialplan very carefully.
