Monitor module in 7609-S

hi all,

do any1 knows, which command will be used to monitor snmp of module WS-X6748-SFP in 7609-S

i have proper snmp configuration in place but when my module went down, i got no laert in LMS3.0.1, instead i got alerts for access switches connected on that module

any help is appreciated

below is the config....on 7609
