WLC 4402: Added 4 1250 APs fine, cannot add 5th one

Please help.

WLC 4402 software version: 6.0.188, licenced for 12 APs.

I have five Cisco 1250 lightweight APs.  Added 4 successfully to the controller, the 5th one will not add and get the following errors on the controller:

*Apr 15 17:08:55.407: %CAPWAP-3-AP_DB_ALLOC: capwap_ac_db.c:145 Unable to alloc AP entry in database for 172.16.31.5:30798
*Apr 15 17:08:55.407: %LWAPP-3-AP_DB_CREATE_ERR: spam_lrad.c:5183 Unable to create an entry for AP 00:00:00:00:00:00 in the database - invalid mac address

I have done full reset of this access point, but still no luck.

%LWAPP-3-AP_DB_CREATE_ERR: spam_lrad.c:5183 Unable to create an entry for AP 00:00:00:00:00:00 in the database - invalid mac address

If your AP does not start with a MAC address of 00:00 then it's a known bug.  This bug was rectified following the release of the 6.0.196.0 code.

I have now applied code 6.0.196, but erro below is still occuring:

Any other ideas?

After resetting 1250 AP I also get error appearing in the WLC 4402 logs:

*Apr 16 14:35:54.824: %WEB-3-INVALID_TFTP_DNLDCODE: file_download.c:893 TFTP download failed with unknown failure code.

I am seeing this same error on a 1242 AP on my 4402 controller that I just upgraded to 6.0.196.  were you able to resolve your issue?

I resolved this by changing the IP the AP gets allocated.  It is has now been added to the WLC 4402.
