I downloaded the Attendant Console Plugin. When I click on the icon to launch the program I am prompted with a box to enter the ip address of the call manager which I do. When I click on save I get an error box stating unable to connect to server. The default server port is 2912.

Make sure the AC services are up and running.
Also check for firewalls/antivirus on your PC and in the network path between the AC client and CUCM server.

Is my Username and Password determined from my End User page? or
from the AC applicattion user page?

It's an AC user you should have created for this purpose.

I'd suggest you to review the whole guide to make sure your config is good.
