Attendant Console Transfer to VM

When trying to transfer a call from AC to voicemail I get the following error: (failed to transfer to voicemail).

I am on CUCM 6.1.1 Unity 5.0

Is there a patch for this fix?

Does the AC phone/line has a valid VM profile (other than None)

Transfer works from phone? without using the AC application?

New install or this was working before?
