;
;Start of section 0 - Basic Credentials
;
;HEADER:
management_ip_address,host_name,domain_name,device_identity,display_name,sysObjectID,dcr_device_type,mdf_type,snmp_v2_ro_comm_string,snmp_v2_rw_comm_string,snmp_v3_user_id,snmp_v3_password,snmp_v3_engine_id,snmp_v3_auth_algorithm,rxboot_mode_username,rxboot_mode_password,primary_username,primary_password,primary_enable_password,http_username,http_password,http_mode,http_port,https_port,cert_common_name

Import from file failed. Reason: Invalid file format. Line 7 of the input file has more fields than the HEADER

Try attached file.  The best way to resolve these types of problems is performing a DCR export from the server.  Then, edit this file using Notepad++ to include the devices you want.  Sometimes if you open with Excel it can change the format of the file making it unusable after saving.

to clarify, I'm sure the same number of fields in the HEADER is the same number of field devices

Please post the actual CSV file you are using.

the name of the archive is ciscoworks.csv

this is the archive:
