It can be a number of things but the commonest is that the router didn't find a bootable IOS version. Note an IOS version not the running-config. It can be because it doesn't have an image in flash or more likely that the config register has been set incorrectly.

Hi ,

It means that the router cannot find a valid image in the flash memory. The image usually gets corrupted or lost.

1. Your config-register tells the appliance to boot into ROMmon;
2. Someone performed the intial password recovery (Ctrl+Break) or hold the Mode button during power up;
3. In routers with no boot statement: The first image is either corrupted or an invalid image;
4. In routers with boot statement: IOS file is not found; and
5. In old switches, the boot statement is wrong or no IOS file found

Basically it is the basic boot to the IOS. Like what Leo mentioned, you can break it or if the IOS is corrupted, you will get to this problem too.
