UCM v7.1.3(2) - Mobile Voice Access failing to make calls

I am able to sucessfully login with remote destination number and PIN credentials. I get as far as the prompt 'enter the number you wish to call followed by the pound key'

When I enter a number (internal or external) followed by #, the call immediately disconnects.

Has anybody come across this issue? better still does anybody know how this issue can be resolved?
