Call routing issue...call manager 7.0

Dear All,  I am facing the following two issues.  1. I am using call manager 7.0 and router 2811 with 4 ports fxo card. Four telephone lines are terminted in fxo card. When I dial a number my two ports go off-hook permanently even i disconnect the call. After that I don't be able to make the call on all four numbers. I only could be able to dial on two numbers. Once i removed the cable from port and reconnect it than i will be able to do call on all four numbers.  Telephone lines number are UAN:111111532 99262397 99262398 99262399 99262400 2. Call is not transfering properly to IP phone extension #1003. Whereas on other three number it's transfering properly. Also one telephne line from PSTN to FXO card is dead. And it's terminating in voice-port 0/1/3.

I just rewrite each and everything properly.
