CFA to voicemail not working

This is a CM 7.1.3, Unity Cxn environment.  When callers hit the auto attendant and dial a 4 digit extension, if that person is call fowarding all to voicemail they hear MOH and a sounds as if it is skipping or in a loop.  If you were to dial the person's DID with the same CFA to voicemail you receive their voicemail.

Need a little help with this, first time I've seen this happen.

2 questions before trying to dig into this:

1) Do you have Unity configured to do a "Release to Switch" transfer or "Consultative Transfer"?

2) What are the transfer actions of the user's mailbox set to?  (e.g., ring extension, go to greeting, etc).
