DHCP issue after issue voice vlan command

Dear all,

I have a question here. Recently our company is going to deploy Cisco IP phone 7911 series in the office area. The phone is connected to the Catalyst 4506 switch and it is powered up using the power socket (not connected to the PoE switch). Since we would like to save up our time instead configuring the switch port one by one, we use the interface range comamnd to configure in one shot. Then we only go to each cube and setup the Cisco IP phone. In the mean while we deploy the phone, we face issue from the user that they cannot connect to the network with the "Limited or No Connectivity" message. For those user facing this issue, we have not yet setup IP phone and their desktop/laptop are directly connected to the switchports with below comamnd.

The commands for the port level configuration are as below:-

!
interface FastEthernet5/8
switchport access vlan 45
switchport mode access
switchport voice vlan 721
no logging event link-status
qos trust dscp
no snmp trap link-status
storm-control broadcast level 0.50
storm-control action trap
spanning-tree portfast
spanning-tree bpduguard enable
!

The commands for the uplink port level configuration are as below:-

!
interface GigabitEthernet1/1
switchport trunk encapsulation dot1q
switchport trunk native vlan 1000
switchport trunk allowed vlan 48,92,721
switchport mode trunk
qos trust dscp
udld port aggressive
storm-control broadcast level 0.50
storm-control action trap
tx-queue 3
   priority high
spanning-tree portfast disable
ip dhcp snooping trust
!
interface GigabitEthernet1/2
switchport trunk encapsulation dot1q
switchport trunk native vlan 1000
switchport trunk allowed vlan 48,92,721
switchport mode trunk
qos trust dscp
udld port aggressive
storm-control broadcast level 0.50
storm-control action trap
tx-queue 3
   priority high
spanning-tree portfast disable
ip dhcp snooping trust
!

Before we configured command lines switchport voice vlan 721 and qos trust dscp, everything is working fine. Users can connect to the network until we issue those command lines. It is something wrong with the VoIP command until the desktop/laptop cannot obtain DHCP IP from the server? Or we can just only issue those VoIP command once we have connected the Cisco IP phone to the switchport?

We are going to deploy the same Cisco IP Phone 7911 in other branches also. Since this is the first branch that we have started the Cisco IP phone deployment, so we would like to solve above stated first before we proceed to other branches deployment.

Please kindly advice on this issue we have faced here. Any help from you guys i really appreciated Thanks in advance.

I have this problem too.
