We have a CallManager 4.1(3) and an IOS VG (a 2801 with a 12.4(22)T IOS).
We are experiencing the folloging wrong behavior in incoming calls: it happens that, when a call arrives on an IP Phone, it rings a first time, then it stops (the call seems to be dropped). After 3-4 seconds, it rings again, then it stops. After other 3-4 seconds, it rings for the third time; in this situation, it keeps ringing normally.
On the calling phone, nothing can be heared until the called phone rings for the third time (the "definitive" one): i.e., only when the called phone rings continuosly, the calling phone rings out.
