The UC540 is setup in PBX mode but the FXO lines are directed to the CO lines. Each phone has the CO lines as buttons. When someone picks up a call, the transfer softkey is greyed out. The transfer to vm key works fine. Is there any way to make this work?

This problem is still not resolved.
Here is some more information. When you make an outgoing call, the transfer key is an available option.

Are your CO line dn's configured as dual-line?
Have you also verified the ephone-templates are correctly configured?
If so, you might want to open a case with TAC.

It ended up being a problem with the dn's.
The incorrect line was:
trunk A0
For it to work, it should be:
trunk A0 monitor-port 0/1/0
