CM 4.2 - When I place a call on HOLD then press the Resume button I can't get to the call. The call stays on my phone. Then 20 seconds later the call drops.

If this is for external calls and you use H323 then most likely you need to enable MTP.

I think CM 4.2 supports an ECS for supplementary services, so you may not need an MTP. Also, IOS has h450.2 and .3 on by default, which CM doesn't support. Try configuring this under the gateway:
voice service voip
no supp h450.2
no supp h450.3
h323
emptycapability
