Conference Causes Dropped Calls

I've got a user that when he attempts an ad-hoc conference and attempts the joing the second line the conference, it just disconnects both lines.

What could be causing the conference to disconnect the calls?

Most likely it's a codec issue, check they're all using G711 if using SW CFB or add a XCODER.

If HW CFB make sure relevant codecs are under the dspfarm profile.
