It's too early in the morning for me.  So let me understand your inquiry ... You want to monitor switch ports using the IP address of each ports?  Or do you want to monitor each ports by using ONE management IP address?

If your inquiry is the latter then there are alot of proggie out there that monitor switch ports by using ONE management IP address and using SNMP.

Solarwinds' Engineer's Toolset is my favorite.  CiscoWorks is another.
