Click-to-Call Problem

I am having an issue with click-to-call for one of my users. Her extension can no longer use this application. In preferences there is no phone available to select.. Also her personal directory now looks for a user id and pin. Has anyone seen this before?

Have you been able to compare this user with another user whose account is working properly?  It sounds like you have a broken device association among other things, yes?

Are you doing and LDAP sync or did you manually build the user directory?
