If it's for use with Cisco devices then Cisco do a TACACS/Radius server called ACS which you can get as software for Windows or you can have a dedicated hardware applicance. See this link for more details -

... and if freeware solution is called for then one of the most popular Radius servers (also used professionally) is the FreeRADIUS at http://www.freeradius.org (runs under Linux). Also there is an open source implementation of the TACACS+ server at http://www.shrubbery.net/tac_plus/ (also a Linux software). Both these products work well with Cisco devices.

Any windows alternatives?  I do have a spare windows 2003 server.  If not, I'll go with freeradius.

ACS does work on Windows.

However Microsoft have their own Radius Server called IAS that works on Windows 2003, see this link for more details -
