I found this exact issue being described on VMware's message board below and it's referncing firmware updates.
Has anyone else seen this and what was your fix?

Reviewing the VMWare case, I would review the firmware release of UCSM and CIMC. When upgrading from 1.3 to 1.4, there was a KVM upgrade were the two KVM subsystems were incompatiable. The KVM would show the B&W portion of the ESX boot but would 'freeze' once loaded, the KVM would not show the Y&B portion of the boot. The host still booted but the KVM did not show it.
Once the CIMC and FI were running the same firmware package, this issue ceased.

I don't know which firmware you are running, but I ran into an issue with USB legacy support in the BIOS that would cause ESXi to hang on boot. It didn't do it on all the blades.....
Here is a link that discusses it..... not sure if this is what you are running into this issue or not....
The fix is to disable Legacy USB support in the BIOS.
