B series hosts connected to MDS and SAN not showing up

OK so I just installed a B series chassis with two 6248 FI's connected to two seperate MDS 9124 FC switches.  The MDS switches have been in production for a while and have our SAN as well as other hosts connected to them.  My SAN is setup in passive mode and basically just discoveres all devices connected to the FC switches that are in the same zone.  THe problem I am having is my SAN can see all my blades that are connected to MDS-A but none of them that are connected to MDS-B.   The SAN can see other hosts connected to MDS-B just fine.  I had my cisco guys in here yesterday looking at the config for hours and they say it should be working.  We are at a loss and Im hoping someone here can take a look at the two below configs and point out something we are doing wrong.  In the configs you will notice the new hosts are labeleld 

In the MDS-A config the new hosts are labeled the following and are seen perfectly by the san.  They have the same basic naming conventions on MDS-B

Few things to check,

1. Check that the port on FI-B that connects to MDS-B is configured to use VSAN 100 and that the port is up.

2. Check the service profiles for the ESX servers in UCS have a vHBA configured to use fabric B.

Make sure that NPIV is enabled on the MDS-B switch ports which are connected with FI. Also please provide the output for command #show flogi database.

Also on MDS-B try out the commands fcping "wwpn of blade" "wwpn of storage port".

So it looks like npiv is enabled, 'feature npiv'.

I see the VSAN is the same across both switches, unconventional but workable. Is the VSAN configured on all of the uplink ports of the UCS FI's (typically as a dual fabric VSAN) - there are two steps here; 1. configure VSAN across the UCS, 2. Configure each of the uplink ports with the necessary VSAN.

Is the HBA configuration failover or dual? In a similar fashion to vNIC failover, you might not see the addresses log into the Fabric in this configuration.

Ideally, each FI has it's own MDS, each with it's own VSAN. The MDS' are meshed back to the SAN's. I can confirm that this configuration works with UCS 6120XP each uplinked to a dedicated MDS9148 which are in turned meshed to EMC VNX 5500 SAN, EMC VNX5700 SAN and Storagetek 6540 SAN.

Also, while I think of it, NXOS 4.x is relatively old.Our MDS9148's, which are just over 12 months old, shipped with 5.0.1a. They all got upgraded to 5.2.2d in August this year.

It would be worth checking the NXOS code version VS UCS code revision VS SAN code VS OS/Hypervisor as well.
