Custom Notification Page - send email with block info

I would like to have link on the custom block page that will send an email with the detail of the blocked site so that users can submit it for review.

Anyone have an example of this or suggestions?

1.  I found that using %0D%0A for a carriage return doesn't appear to work.

2.  I need to put variables, i.e. the URL, Category, etc., into the email body.

I should have mentioned this is via the On-Box message under Security Services, End-User Notification, End-User Notification Pages, Custom Message.
