CISCO SF300's losing management access

Recently we have seen a substantial amount of our switches losing management access in our networks for our clients. The switches are up and active and no phone lines are lost. However, when you attempt to telnet into the switches you will not be able to outside of the router (gateway) itself.

Often the switch will need to be rebooted to restore management access but sometimes this does not work and we need to replace the entire switch.

Does this issue occur for anyone else? and if so has anyone found an answer or a cause of this issue to happen?

-Did the problem start happening ever since the switches were implemented in the network?

-What is the firmware version running on the switches?
