Can somebody please confirm that this issue is only seen when upgrading UCSM? or what is the circumstances when Customer will see this issue?

The Adapter to IOM port channel that is getting erased and then recreated during the upgrade, is causing the flap.

Normally we never see these port-channels change except under conditions where you would expect to have a blade connectivity outage such as decommissioning the blade.
