Courtesy call back call is not disconnect

Dear all,

I have done the default CCB setup , now call back is offering but after call back  request call is not disconnected .

Call is waiting for long time , call is not disconnect from Phone .

  

VG : Cisco IOS Software, 2800 Software (C2800NM-ADVENTERPRISEK9-M), Version 15.1(4)M7 

CVP : 11.0.1

Reporting : 11.0.1

Please help us to solve this issues . Please find debug voip application all.
