I was reading about this advisory
CSCvd23476 where it says the POE port suddenly stops providing POE as we are seeing that issue in our company where Avaya phone suddenly stops getting power and go dead. It just started happening and we saw it on few 4510R+E switches having SUP 8-E.

we had the same issue rolling back to 3.6.6 fixed it , its stable ios-xe image on all platforms

I've seen the same issue about four times on our 4510R+E and 4507R+E switches, and we're running the 3.6.6 code. Unfortunately, it's not something that we can easily replicate to determine if a higher software release would fix this issue. The one thing we do know is that it only happens on these specific type of modules.

Forgot to mention that we're using firmware 15.1(1r)SG4 so not sure if Mark is using 15.1(1r)SG5 or a higher version and perhaps that's why he is not seeing issues anymore.

We are running 3.6.6 and have this issue. What version of ROMMON are you running? We are on 15.1(1r)SG5
