When users dial an external phone number,they will sometimes receive a 'fast busy" tone on entering the last digit.The second dialling attempt is always successful.

Sounds like a carrier side problem. Try to catch debugs of failed calls by doing outside calls by yourself.
If you can, it will be possible to find out the exact cause of the failure.
