Netdisco is an Open Source web-based network management tool. Designed for moderate to large networks, configuration information and connection data for network devices are retrieved by SNMP. With Netdisco you can locate the switch port of an end-user system by IP or MAC address. Data is stored using a SQL database for scalability and speed. It also provide optional use of the Cisco Discovery Protocol (CDP).

I cannot recommend Nagios enough for this. I doubt you will find many that think Nagios isn't one of the most powerful monitoring frameworks available for free now. While Nagios does not provide "out-of-box" monitoring functionality (give me an IP and I'll monitor all the services on it), it can monitor absolutely anything that can be checked via external scripts. We monitor over 1000 nodes using Nagios and do everything from basic pinging and interface status checks, to router filesystem monitoring. Check out www.nagios.org and don't let it's massive configuration scare you. I guarantee that if you take the time and learn Nagios, you'll find few free monitoring platform that out-perform it.
