Problem Descrition:
When you try to install Vmware ESXI 4.1 on UCS C serise or B serise server. if you see this error
"Vmkctl.HostCtlException: Unable to load module /usr/lib/vmware/vmkmod/vmfs3:failure"
Resolution:
Need to download Cisco Custom image from this Link on VMWare site

Upgrading Esxi
