Are you running at least IOS 15.1(1)T1? I believe there was a caveat running 69XX phones in SRST on IOS 15.1(1)T and below. Also which firmware are the 6911's running.
By chance is the SRST configuration in CM set to "Use Default Gateway"? There was a bug for this a little while back.

All the phones are in the same device pool?
did o try to research this issue on bug toolkit?

The phone 7941 and 6911 are in the same Device Pool.
The IOS of the Gateway is:
Cisco IOS Software, C2900 Software (C2900-UNIVERSALK9-M), Version 15.1(3)T1, RELEASE SOFTWARE (fc2)

Can not use the default gateway as SRST
The IOS looks good, some of the older 15 trains had some strange issues with SRST and the 69XX phones.
Which firmware are the 6911's running? Do they meet the requirements in the SRST 8.5 release notes.

The Device Defaults of the CUCM:
Device Type: Cisco 6911
Load Information: SCCP6911.9-0-2-0
Cisco Unified CM: System version: 8.0.3.20000-2
In the web administration of the phone (http://ipaddress6911), I have this information:
App Load ID
6911SCCP.9-0-2-0
Boot Load ID
6911.0-0-0-01-02
01-18-2012 07:19 AM
According to the SRST Release notes, you need SCCP6911.9-1-1-0.loads for SRST 8.5

Mathew
Look at the bug I sent
Found in 9.0.2 (YOURS)
Fixed in 9.1.1
Do you use the DEFAULT-GATEWAY as the SRST reference or IP Address
