If you have a Cisco ACS appliance running 4.0, can you have a 4.0 version of ACS running on a standard server for redundancy, or do they both have to be appliances?

You can indeed mix and match sw & appliance versions of ACS.
Replication should even work between them - although I've never tried this to know it actually definately does work.

Yes, but if your parent is a Windows Server and a child is an applinace, your child's NT Agents will disappear upon Network Device Replication since the parent doesn't have NT Agents. You'll either have to manually add them after replication, or disable Network Device replication. The workaround is to manually add the devices to the parent and the child and just skip that piece of replication.
