1)The newer version ACS is ACS 5.1 and runs on a Cisco ACS 1121 hardware.
More from the release notes here :
Otherwise you can also build a VMWare machine to install it on :
2 and 4)
The migration documentation is from version 4.x to 5.1 and is available here :
There will be a lot of manual intervention and analysis involved in the migration.
Take note the following :
The most efficient way to make optimal use of the new policy model is to rebuild policies by using the new policy model. This method entails creating appropriate Identity Groups, Network Device Groups (NDGs), conditions, authorization profiles, and rules.
3) It is a linux based appliance.

Hi Tan,
Following is our upgrade path,
1) 3.3.3-->4.1
2) 4.1 -->4.2
Before you start upgrade process make sure to take a backup of your ACS configuration.
Here are the steps to upgrade ACS windows, On server take a backup for 3.3 and keep it handy.
1) Run the setup of 4.1 During this process, it will prompt you to upgrade existing configuration. Make sure you check that option else we will loose the database. Now you need to hit next.next to finish the 4.1 upgrade.
2) Now on 4.1 take a backup and keep it handy. Run the setup of 4.2 During this process, it will prompt you to upgrade existing configuration. Make sure you check that option else we will loose the database. Now you need to hit next.next to finish the 4.2 upgrade.
You can use Win2003 (Migration machine) for above.

~JG
