Hello,
For the username you can try using admin or cisco along with the password you set, it should be one of those two.
If that isn't working for you there is a factory reset option available in the phone's on-screen menu.
Once the phone is powered on press the button that looks like a piece of paper with a folded corner.
From there scroll through the menus until you find Device Administration, and then select Factory Default, and confirm. The phone will reboot and will come back up at factory defaults.
