Sipura 1001 - No Dial Tone - No Web - Trying to do a factory reset

I have a Sipura 1001 ATA and am trying to do a factory reset by phone since the unit has an unknown or no known IP address.

Steps as per documentation

1) Dial ****

2) 73738#

3) Confirm with 1, as necessary

Issue: No dial tone to reach IVR in the first place. IVR does not respond. Punching in the digits without confirmation yields nothing. Tried various timing patterns to no effect.

Can't get to it by IP, so I was watching the DHCP server logs to see if the unit requests an IP, which never happens.

Blink codes:

Orange (solid)

Green (blinking in half second intervals for 12 secs, then resets)

Tried plugging in only the phone and no Ethernet, or only Ethernet and no phone.

Suggestions?

It sounds like the unit is damaged.
