This issue can occur when a Cisco CallManager is recently upgraded, and the Bulk Administration Tool (BAT) tool possibly was not upgraded.
To resolve this issue, perform these steps:
Go to Application > Install Plugins > Cisco Bulk Administration Tool. Click on Cisco Bulk Administration Tool to start the installation.

3. You might also need to recreate the phone templates to make it work. The phone templates are different once you install BAT from the plugins.
You need to use the new version of BAT.xlt tool in order to create the CSV file.
