agent issues

We are having problems with the phones once again. One phone is stuck on requesting(wheel of death) we force quit that one, and the other phone is not allowing our agent to take calls. He forwarded a caller to x1040 and after he did that the screen froze with the message "forwarded to 1040".

At that point my agent tried to log out since it would not allow him to take calls, we force quit it, he logged into another phone and it prompted him with the same 1040 message. Although there were 2 people in the queue he was still unable to take calls, he would place himself on "ready" and the phone would place him on "not ready." Right now he is unable to take calls regardless of where he logs in.

Help please.
