CAD 7.6.3 Not able to dial out using Dialpad

When trying to place an outgoing call from the dialpad, once the number is entered the dialpad dissappears.  Has anyone seen this before? Logs say CAD cant see the phone??  any imput or troubleshooting suggestions would be most helpful.

is it UCCX or UCCE ? what version ?

What is the OS on which CAD is installed ?

Also, attach those logs which you saw.
